Job Summary:
The Case Manager is responsible for assessing client needs, developing individualized service plans, and coordinating services to support clients in achieving their goals. This role involves direct client interaction, advocacy, and collaboration with other service providers to ensure comprehensive care.
Key Responsibilities:
- Conduct client intake, assessment, and eligibility evaluations.
- Develop and implement individualized service plans tailored to client needs.
- Provide case management services, including referrals to community resources, counseling, and advocacy.
- Monitor and document client progress, adjusting service plans as needed.
- Collaborate with community organizations, social service agencies, and government programs to coordinate services.
- Maintain accurate and timely documentation in compliance with organizational and funding requirements.
- Assist clients in navigating challenges such as housing, employment, healthcare, and legal issues.
- Facilitate support groups, workshops, and educational programs as needed.
- Advocate for client rights and access to essential services.
- Participate in staff meetings, training, and program development initiatives.
Qualifications:
- Bachelor’s degree in Social Work, Psychology, Human Services, or a related field (Master’s degree preferred).
